29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous. a. In the building under construction, next to the entry way, the flexible cord set used to energize the light was missing the grounding pin. The employees were exposed to contact with electrical current hazard.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables shall be protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage. a. Inside the building under construction, the Hitachi C10FS Miter Saw had damaged flexible cord. The employees were exposed to contact with electrical current. b. Inside the building under construction, the employees were using a cord set to power a light. The external insulation for the cord set and the strain relief were damaged. The employees were exposed to contact with electrical current.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. At the worksite, employees were observed working on the roof of a new commercial building without fall protection. The employees were installing metal roofing and were exposed to a fall hazard of 12 feet.
