29 CFR 1926.453(b)(2)(v):   A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:   This violation occurred on or about November 26, 2014 at 110 E. Broadway, Altus, Oklahoma, Home Again Remodeling, LLC did not ensure each employee was wearing a safety harness with lanyard attached to the proper anchor points while in the aerial lift platform on the Ford F700 exposing employees to a fall from approximately 10 ft to the ground below.
