29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  At the time of the inspection, the machine specific procedures did not require the use of locks by employees conducting maintenance activities on equipment such as the horizontal baler, vertical baler, and cutters.     Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

29 CFR 1910.215(a)(4): Grinding machinery was not used with work rest(s) to support offhand grinding work:  At the time of the inspection, the Ashley bench grinder was used and was not equipped with work rests or tongue guards, exposing employees to a struck by hazard.     Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

29 CFR 1910.22(d)(1): In every building or other structure, or part thereof, used for mercantile, business, industrial, or storage purposes, the loads approved by the building official shall be marked on plates of approved design which shall be supplied and securely affixed by the owner of the building, or his duly authorized agent, in a conspicuous place in each space to which they relate. Such plates shall not be removed or defaced but, if lost, removed, or defaced, shall be replaced by the owner or his agent.   At the time of the inspection, the storage area over the main office was not marked with the approved load limit rating.
