29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.      a) At the jobsite: 2309 Coral Street, first floor - Employees were not protected from falling through a stairway opening while working on the first level.  The stairway opening was connecting the first floor with the basement and exposed employees to a fall hazard of approximately 6 feet and 6 inches. Observed on or about 11/20/14.    b)  At the jobsite: 2309 Coral Street, second floor - Employees were not protected from falling through a stairway opening while working on the second level.  The stairway opening was connecting the second floor with the first floor and exposed employees to a fall hazard of approximately 14 feet and 6 inches. Observed on or about 11/20/14.
