On or about November 12, 2014, at least one employee operating a pneumatic nail gun to perform decking activities was not wearing eye protection exposing him to being struck by flying material and eye injuries.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system.  On or about November 12, 2014, employees performing framing activities on a residential framed structure were exposed to an unprotected fall hazard at a height of over 6 feet to the ground below.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:  On or about November 12, 2014, employees performing framing activities on a new residential home were not provided a ladder to safely gain access to the upper areas exposing the employees to an unprotected fall of up to 11 feet to the ground below.
