29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers:   a) At the jobsite: 30 S. 15th Street, Graham Building, 15th and 16th floors - Employees engaged in renovation operations and in proximity to floor holes were exposed to fall hazards of approximately twelve (12) feet without the use of a guardrail system, a safety net system, or personal fall arrest systems.
