29 CFR 1926.605(c)(3): The employer did not prohibit employees from passing fore and aft, over, or around deckloads, unless there was safe passage.     On or about December 1, 2014, employees were exposed a fall hazard into the Fox River, while passing fore and aft and around deckloads, and the employer did not ensure a safe passage way was provided, while performing the following activities:    a) Employees were required to pass around the hydraulic power pack control station, located along the east side of the barge deck, to gain access to the controls.      b) Employees were required to pass fore and aft the east side of the barge deck, adjacent to the crane outriggers, to access tools.      Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

On or about December 1, 2014, the employer did not ensure that all outriggers were fully extended on the barge-mounted, Terex RT 555, as required by manufacturer set up foot note #1.  This condition exposed employees to a struck-by hazard.    Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
