29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a) Worksite: Employees working directly under pump jack scaffold installation were not using head protection; on or about 12/4/2014.      Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

29 CFR 1926.451(g)(2): The employer did not provide fall protection for employees erecting or dismantling supported scaffolds:    a) Worksite: Employees were erecting a pump jack scaffold to do siding work from the roof of the residential building approximately 18' above the ground and were not protected from falling; on or about 12/4/2014.    Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) Worksite: Employees were accessing a roof approximately 16' above the ground to install a pump jack scaffold for siding work. The portable extension ladder used to access the roof extended approximately 12" below the landing surface; on or about 12/4/2014.    Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
