29 CFR 1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area.  Worksite: The employees who were working from the pump jack scaffold that was not erected properly and without fall protection at a height above 10 feet were not trained to recognize the hazards associated with the type of scaffold being used.

29 CFR 1926.452(j)(2): Poles shall be secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. When the pump jack has to pass bracing already installed, an additional brace shall be installed approximately 4 feet (1.2 m) above the brace to be passed, and shall be left in place until the pump jack has been moved and the original brace reinstalled.    Worksite: The pump jack scaffold that employees were working from was not braced or secured at the bottom of the scaffold poles.
