29 CFR 1926.602(a)(9)(ii): Earth moving or compacting equipment which had an obstructed view to the rear was operated in reverse gear; such equipment did not have in operation a reverse signal alarm distinguishable from the surrounding noise level or did an employee signaled that it is safe to do so:      North East parking lot: On or about 11-26-2014, employees were exposed to equipment backing injuries while doing fine grading around  a Caterpillar grader that did not have an operational back-up alarm and did not have a designated ground guide to ensure the safety of the employees working around the grader.      Note: Abatement Certification is required for this item.
