29 CFR 1926.651(b)(4): While the excavation was opened, underground installations were not protected, supported or removed as necessary to safeguard employees: a) Fort Belvoir Army Base: On or about 12/04/14, an employee was working in an unprotected trench adjacent to an unsupported underground utility pipe and excavator bucket, thus exposing the employee to possible crushing injuries. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.651(c): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees: a) Fort Belvoir Army Base: On or about 12/04/14, an employee was working in a 4.5 feet deep excavation without a safe means of egress, thus exposing the employee to injuries. No abatement certification or documentation required.

29 CFR 1926.651(j)(2): Employees were not protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations by placing and keeping such materials or equipment at least 2 feet from the edge of excavations, or by the use of retaining devices that are sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary: a) Fort Belvoir Army Base: On or about 12/04/14, the spoil pile was not at least two feet from the edge of the trench while the employee worked inside the trench, thus exposing the employee to injuries. No abatement certification or documentation required.
