29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury:  On or about December 9, 2014, the employer did not provide training for four employees performing activities including, but not limited to, working with electricity; employees were exposed to electrical shock hazards.

29 CFR 1926.403(b)(1)(i): Safety of equipment was not determined to be suitable for installation and use in conformity with the provisions of this subpart:  On or about December 9, 2014, employees used an ungrounded hand-made electrical handy-box (Carlon 32 cu. in. electrical indoor box) with two duplex receptacles (15A, 125VAC, 2-pole, 3-wire) to operate their power tools. The handy box attached to Romex (indoor only) 10/3 NM-B was wired directly to the citys electrical utilities box. Employees were exposed to electrical shock hazards.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  On or about December 9, 2014, the path to ground from circuits and equipment were not permanent and continuous exposing employees to electrical shock hazards in the following instances:    a)   Employees used an ungrounded hand-made electrical handy-box with two duplex receptacles (15A, 125VAC, 2-pole, 3-wire) to operate their power tools.    b)  Employees used an 110V yellow extension cord with the ground pin missing to operate electrical equipment.
