29 CFR 1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l):    29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    Construction Site: on or before December 9, 2014, and at times thereto, an employee was operating a power industrial truck and was not trained or evaluated. This condition exposed the employee to fall, caught-by and/or struck-by hazards.

This condition exposed the employee to fall and crush-by hazards.
