29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.  At the time of the inspection, the following area were not equipped with standard guardrails or fall protection;  a) The mezzanine above the office was missing portions of the top rail and mid-rail exposing employees to a fall of greater than 8 feet to the concrete floor.  b) Truck drivers were operating the truck mounted loader from a work platform greater than 12 feet above the ground with no guardrails or seat belts to prevent a fall.

29 CFR 1910.305(b)(1): Unused openings in boxes, cabinets, or fittings were not effectively closed;  The walk around found multiple opening in main electrical panels and cabinets along the north wall of the warehouse. The openings were exposing employees to live electrical up to 440 volts.

29 CFR 1910.1200(g)(1): The employer did not have a safety data sheet(SDS) for each hazardous chemical in use;  At the time of the inspection, the employer had no SDS for hazardous chemicals such as but not limited to; gasoline, Ranger herbicide, Brite Alume cleaner, and paint thinner.
