John Parks DBA Parks Roofing worksite located at 400 Dixie Hwy., Rossford, Ohio: On or about December 10, 2014, the employer did not ensure employees were protected from head injuries while working below employees performing weatherproofing activities. Due to this condition, employees were potentially exposed to impact injuries caused from falling material and/or equipment.

John Parks DBA Parks Roofing worksite located at 400 Dixie Hwy., Rossford, Ohio: On or about December 10, 2014, the employer did not ensure employees were provided with proper eye protection while operating pneumatic nail guns to secure asphalt shingles. Two employees were exposed to potential flying object hazards while operating the pneumatic fasteners.
