29 CFR 1910.304(f)(1)(iv): Overcurrent devices for circuits rated 600 volts, nominal, or less, were not readily accessible to each employee or authorized building management personnel: Establishment: Overcurrent devices were blocked by items in storage: a) Maintenance Shop - Disconnects for equipment such as pump #1 and pump #2 for the irrigation system were blocked by a flammable storage cabinet. b) Boiler Room - Overcurrent devices were blocked by items in storage such as housekeeping carts, storage bins, cardboard boxes and various refuse. c) Kitchen - a circuit panel (overcurrent device) was blocked by a storage bin of soiled kitchen cloths.

29 CFR 1910.138(b): The employer did not base selection of appropriate hand protection on an evaluation of the performance characteristics of the hand protection relative to the tasks to be performed, conditions present, duration of use, and the hazards and potential hazards identified: Establishment; Employer did not select gloves that would provide adequate protection during the following operations: a. Laundry Department - employees who use hazardous materials such as Reliance disinfectant bleach. b. Housekeeping - employees who use hazardous materials such as 20 Neutral disinfectant cleaner and disinfectants such as ViraSept.
