29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy: 89 Guion Street, Springfield, MA. On or about December 11, 2014: employer failed to provide energy control procedures for the Grey Guillotine cutter located in the Bollegraaf Baler Room. These procedures failed to identify all specific steps to be followed by employees to isolate and disable the guillotine, to safely de-energize the equipment and control hazardous energy. The procedure also lacked specific steps to ensure the blade was properly blocked in case of a sudden loss of hydraulic power.
