In the warehouse area at 4 Federico Drive Pittsfield, MA: On or about December 12, 2014, storage rack supports were stored in front of the sole exit reducing the width to approximately 12 inches and 21 inches on either side of the supports.

On a maintenance technician truck at 4 Federico Drive Pittsfield, MA: On or about January 20, 2015, the employer did not ensure that damaged personal protective equipment was not used in that a 6 foot shock absorbing lanyard with frayed stitching was used for fall protection by a field technician.
