29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a) For employees working at 200 Gulf Drive in Bradenton Beach, as observed on or about December 11, 2014. Employees were installing plywood roof sheathing on roof with a 1/8:12 pitch. The eave to ground height measured approximately 11 feet 4 inches and the employees had no fall protection system in place.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:   a) For employees working at 200 Gulf Drive in Bradenton Beach, as observed on or about December 11, 2014. Employees were observed standing on the top of a stepladder while performing overhead work.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection.  a) For employees working at 200 Gulf Drive in Bradenton Beach, as observed on or about December 11, 2014. Employees were using extension cords to supply power to electrical tools. The extension cord was connected to a temporary power supply that did not have a ground fault circuit interrupter for personal protection.

29 CFR 1926.403(b)(1)(i): Safety of equipment was not determined on the basis of suitability of the equipment for an identified purpose as evidenced by listing, labeling, or certification for that identified purpose.   a) For employees working at 200 Gulf Drive in Bradenton Beach, as observed on or about December 11, 2014. Employees were using power tools that were supplied power from temporary power taps. The temporary power taps were job built 4inch metal boxes with 4 outlets installed powered by 12/3 solid copper romex wire.
