29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about December 11, 2014, and at times prior thereto, the employer did not ensure that an employee installing sheathing from a 2nd floor balcony was provided with fall protection. The employee was exposed to a fall hazard of a height of over 6 feet to the ground below.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:    On or about December 11, 2014, and at times prior thereto, an employee installing sheathing from a 2nd floor balcony used an "A" framed ladder in the closed position against a wall to access upper levels exposing the employee to a fall hazard.
