29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a)  At the jobsite, while installing trusses, on or about 12/8/14:  Employees were working approximately 20 feet from grade without fall protection.  b)  At the jobsite, while installing trusses, on or about 12/8/14:  Employees were working at 9 feet above a lower level without fall protection.  Abatement certification must be submitted for this item.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)  At the jobsite, while installing trusses, on or about 12/8/14:  Employees were accessing the truss installation area of the building with a 6 foot step ladder that did not extend at least 3 feet above the upper landing being accessed.  Abatement certification must be submitted for this item.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:  a)  At the jobsite, while installing trusses, on or about 12/8/14:  Employees were accessing the truss installation area of the building with a 6 foot step ladder that did not extend at least 3 feet above the upper landing being accessed and causing the employees to use the top of the ladder to access that surface.  Abatement certification must be submitted for this item.
