29 CFR 1926.502(b)(1): The top edge height of the top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level:     a) On or about November 18, 2014, employees were exposed to fall hazards while working from the second story balcony at 3936 Georgina Lane where the top edge height of a guardrail system's top rail was approximately 36 inches.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.502(b)(3): Guardrail systems were not capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied within 2 inches (5.1 cm) of the top edge, in any outward or downward direction, at any point along the top edge:      a) On or about November 18, 2014, the employer cut and unsecured the west side guardrail of the second story balcony at 3936 Georgina Lane exposing employees to fall hazards, including a general contractor employee who fell through the unsecured guardrail.     No abatement certification or documentation required for this violation.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:     a) On or about November 18, 2014, employees were exposed to a 13 foot fall hazard while mounting and dismounting an extension ladder positioned against the exterior edge of a guardrail on the second story balcony of 3936 Georgina Lane where there was a break in elevation of approximately 36 inches between the top of the guardrail and the point of access to/from the balcony deck.  Stairways or ladders were not provided between the balcony deck and the extension ladder.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
