29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: The employer is failing to protect employees from laceration hazards by not properly guarding equipment. The work rest on the bench grinder in the maintenance shop was not adjust adjacent to the wheel to the required 1/8" gap. The gap measured at approximately 1/4".

29 CFR 1910.215(d)(3): The contact surface(s) of wheel(s), blotter(s) or flange(s) on grinding machine(s) were not flat and free or foreign matter: The employer is failing to protect employees from struck by hazards due to the equipment not being properly maintained. The surface of the grinding wheels on the bench grinder are worn and grooved so the wheel is distorted from its original shape, instead of being square.
