On or about December 12, 2014 and in times prior thereto, strain relief on 480 volt electrical cords' plugs was not provided, exposing employees to the hazards of electrical shock and fire: a: Power cord to dog house, located on main electrical distribution trailer. b. Power cord to transformer, located near the company trailer.

29 CFR 1910.307(c)(1): Equipment and associated wiring was not approved as intrinsically safe when used in any hazardous (classified) location: On or about December 12, 2014, located under the mud tanks, where subject to ignitable vapors, the wrong covers were used on Cooper explosion proof switches. As a result, the screws used to attach the covers did not fit and were missing, exposing employees to the hazards of fire and explosion.
