29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  a) At the Royal Palm movie theater in Bradenton, two employees wearing personal fall arrest system without being tied off. Employees were exposed to a fall hazard of over 20 feet, as observed on or 12/12/14.   b) At the Royal Palm movie theater in Bradenton, one employee using the aerial lift without personal fall arrest system. The employee was exposed to a fall hazard of over 20 feet, as observed on or 12/12/14.
