A. Tony Deanda DBA Deanda Construction worksite located at 5510 Heatherdowns Blvd., Toledo, Ohio: On or about December 15, 2014, the employer did not ensure employees were provided with proper eye protection while operating pneumatic nail guns to secure asphalt shingles. Employees were exposed to potential flying object hazards while operating the pneumatic fasteners.

Tony Deanda DBA Deanda Construction worksite located at 5510 Heatherdowns Blvd., Toledo, Ohio: On or about December 15, 2014, the employer did not ensure employees were protected from falls while applying new roofing material to a low sloped multi-family structure. Employees were exposed to a fall distance of approximately 18.00 feet.
