29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)    a. Site- The employer failed to ensure that a competent person knowledgeable in the subject of safe electrical work practice including lock out/tag out procedures, required personal protective equipment, and voltage testing procedures conducted frequent and regular inspections of the jobsite where an employee was exposed to electric shock and arc flash hazards. On or about October 27, 2014 an employee sustained an electric shock and fell off of a step ladder when they contacted live power operating at 277 volts.
