29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):   On or about December 10, 2014 and prior thereto at Bekaert Corporation located at 1881 Bekaert Drive, Van Buren, Arkansas, the employer had not assessed jobs tasks such as stripping floors using chemicals such as sodium hydroxide, 2-butoxyethanol, monoethanolamine and cleaning with chemicals such as hydrogen peroxide,  to determine if hazards were present which would necessitate the use of personal protective equipment.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:   On or about December 10, 2014 and prior thereto at Bekaert Corporation located at 1881 Bekaert Drive, Van Buren, Arkansas, the employer had not developed a written hazard communication program for employees working with or exposed to chemicals and substances such as lead, 2-butoxyethanol, monoetanolamine, sodium hydroxide, acrylic polymer, alcohol ethoxylate,  hydrogen peroxide and quaternary amine.
