Bags, containers, bundles, etc., stored in tiers were not stacked, blocked, interlocked and limited in height so that they were stable and secure against sliding or collapsing:       At the worksite:  On or about December 16, 2014, the employer did not have the pipe on the pipe racks chocked or block to prevent shifting.  Employees were exposed to struck-by and caught in between hazards.

29 CFR 1910.37(a)(3):  Exit routes were not free and unobstructed:      At the worksite:  On or about December 16, 2014, the employer allowed employees to conduct work activities from the derrick work platform on a oil/gas drilling rig without having access to the secondary exit route due to the wind curtain blocking access to the escape device.  Employees were exposed to fire and explosion hazards.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm from fall from heights hazards:    At the worksite:  On or about December 16, 2014, the employer allowed the use of an emergency escape device system (Geronimo) that was not installed per the manufactures requirements.  An employee was exposed to the hazard of fall from heights.
