29 CFR 1926.451(a)(1): Each scaffold and/or scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it:    (a)  The outrigger bracket scaffold employees were working from was not capable of supporting its own weight and at least 4 times the maximum intended load being applied to it. This created a scaffold collapse hazard.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.    (a) An employee working from an outrigger bracket scaffold was not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 20 feet.

29 CFR 1926.454(b): The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question.    (a) Employees working from an outrigger bracket scaffold had not been trained to recognize the hazards associated with scaffold work by a competent person in the erecting and maintaining of said scaffold.
