29 CFR 1910.1030(d)(3)(ii): The employer did not ensure that the employee used appropriate personal protective equipment when there was occupational exposure:  a) Facility - On or about 12/18/14, employees work with biohazards, collection and evaluation of biohazards and clean equipment which contains and is contaminated by biohazards.  The employer has identified and documented that goggles are required during certain procedures, such as those above.  The employer did not ensure that employees who were engaged in the identified tasks wore goggles, as required.  ABATEMENT CERTIFICATION REQUIRED
