29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:  a)     On or about December 15, 2014, the employer, Sentry Siren, Inc., did not ensure employees, who were required to access the above-office storage platform, were adequately protected from fall hazards of approximately 9.5 feet.  b)     On or about December 15, 2014, the employer, Sentry Siren, Inc., did not ensure employees, who were required to access a platform above the shop bathroom for cleaning purposes, were adequately protected from fall hazards of approximately 10 feet.
