29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose; and each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:  On the pole next to the Sinclair Rip-Saw an electrical junction box did not have a protective cover installed. Employees were exposed to a shock hazard.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  In the shop employees wore disposable paper masks that were not required by the employer and they had not been provided the information in Appendix D of 29 CFR 1910.134.
