29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):     a)  East side of the Building  - on or about December 17, 2014, two employees were exposed to fall hazard while performing work from the 3rd level of a tubular welded frame scaffold.  This section of the scaffold was not fully planked, exposing the employee up to an 18-foot fall hazard.

29 CFR 1926.451(e)(1): Where scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers, stairway-type ladders, ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used for access:    a)   East side of the Building  - on or about December 17, 2014, two employees were observed working from a tubular welded frame scaffold without a ladder or any other means of safe access/egress to reach the top level of the scaffold, exposing him up to a 18-foot fall hazard.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:    a)   East side of the Building  - on or about December 17, 2014, the employer did not provide adequate training to a temporary employee on how to recognize hazards associated with scaffolds and/or how to reduce or eliminate those hazards,  exposing him up to an 18-foot fall hazard.
