29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about 18 December, 2014, at the above addressed job sites, four employees were working from a roof of a building under construction without the use of fall protections, exposing the employees to a fall hazard of approximately 14 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about 18 December, 2014, at the above addressed jobsite, four employees were utilizing a aluminum extension ladder to access the roof without extending the ladder at least 3 feet above the upper landing surface, exposing the employees to a fall hazard of approximately 12 feet.
