Choice One Carpentry worksite located at 2931 Navarre Ave., Oregon, Ohio: On or about December 17, 2014, the employer failed to ensure the worksite was inspected to identify potential hazards. Employees were exposed to hazards including but not limited to; fall hazards and scaffold hazards. Employees were exposed to multiple fall hazards on the site.

Choice One Carpentry worksite located at 2931 Navarre Ave., Oregon, Ohio: On or about December 17, 2014, the employer did not ensure employee working from a tubular welded frame scaffold were trained to recognized and avoid hazards associated with this type of scaffold, such as fall hazards related to providing scaffolding safe access and fall protection.

The exposed employee was landing material on the West end of the structure exposed to a fall distance of approximately 19.00 feet. No fall protection was being used.    B. Choice One Carpentry worksite located at 2931 Navarre Ave., Oregon, Ohio: The employer did not ensure employees were protected from fall hazards while performing framing activities. The exposed employee was exposed to a fall distance of approximately 19.00 feet while framing on the Southeast corner of the structure. No fall protection was being used.
