Choice One Carpentry worksite located at 2931 Navarre Ave., Oregon, Ohio: On or about December 17, 2014, the employer failed to ensure the worksite was inspected to identify potential hazards. Employees were exposed to hazards including but not limited to; fall hazards and scaffold hazards. Employees were exposed to multiple fall hazards on the site.

A.
