29 CFR 1910.219(c)(4)(i):   Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft:  Employees working at or around the pellet machine were exposed to a projecting shaft end that was not guarded.  The shaft projected approximately 4 inches and the diameter was approximately 1.25 inches.

29 CFR 1910.272(j)(1):   The employer did not develop and implement a written housekeeping program that established the frequency and the method(s) determined best to reduce accumulations of fugitive grain dust on ledges, floors, equipment, and other exposed surfaces in the grain handling facility:  Employees were exposed to fire hazards within the facility because the employer did not implement a housekeeping program that established the frequency and methods to be used when cleaning up fugitive grain dust.

29 CFR 1910.307(c)(2)(i): Equipment in a hazardous (classified) location(s) was not approved for the ignitable or combustible properties of the specific gas, vapor, dust, or fiber that was or could be present:  Employees were exposed to fire and explosion hazards while working near the mixer in the production area because the lighting fixture did not have a dust proof cover and in that condition was not approved for use a hazardous location.
