Huntington WV Area Habitat for Humanity, 6809 Riverlick Ave. Huntington WV:  On or about 12/17/14, an employee performing roofing operations was not protected from falling while working 16' above the ground.    ABATEMENT CERTIFICATION IS REQUIRED.
