29 CFR 1926.454(c)(3): Inadequacies in an employee's work involving scaffolds indicates that the employee has not retained the requisite proficiency. a) 2303 Western Ave Guilderland NY - On or about December 12th, 2014 - An employee was exposed to fall hazards while using two aerial lifts. A personal fall arrest system was not worn and/or a lanyard was not attached to the boom or basket when working from the aerial lifts. The employee was trained in the use of aerial lifts yet the use of the aerial lifts indicates that the employee did not retain the requisite proficiency.
