29 CFR 1926.303(c)(2): Floor or bench mounted grinder(s) were not provided with work rests:   On and before 12/22/14, a Buffalo 8" bench grinder used to deburr EMT conduit was not equipped with a work rest on the right side and the work rest on the left side was not adjusted to within 1/8" of the wheel to prevent material being pulled between the abrasive wheel and the wheel cover.

29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  On 12/22/14, electrical components were used in applications that they were not listed or labeled for including:  a) In the jobsite conference room, the employer had a 4 inch gang box that was used as a pendant box.  The gang box was not listed and labeled to be used as a pendant box.   b)  Located in the basement of the project and next the electrician's work bench, a 220v range plug connector designed to be permanently mounted to wall structures was not secured and used as a cord and plug to welding equipment.

29 CFR 1926.416(e)(2): Extension cords were fastened with staples, hung from nails, or suspended by wire:   On 12/22/14, the electric cord conductors used for temporary electric lighting throughout the project were suspended by using uncoated tie wire creating damaged to electrical cords.
