29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.  (a) Office Area - The employer did not ensure that all places of employment were maintained in an orderly and clean condition. Propane tanks were observed stored on the floor near the entrance door, one tank was used to prop the door open and extension cords were observed laying on the floor throughout the worksite exposing employees to tripping hazards.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a) Office: The employer failed to ensure that space heaters were used in accordance with instructions included in the listing or labeling. Space heaters intended for household use were being used in the production area and were not plugged directly onto a grounded outlet box.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  (a) Office Area - The employer did not ensure that flexible cords were visually inspected prior to use on any shift for external defects or damage. A frayed extension cord with tape wrapped around live wires was used to power a space heater.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
