29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: The Hydrosorb operator was exposed to eye injuries from suspended dust when setting up and unjamming the equipment in the "Powder Room" (HYD-003) because the prescription lenses with safety side shields provided are not adequate to protect him while performing his tasks.

29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form: The employer did not evaluate the respiratory hazards of non-routine work tasks such as the unjamming operations of the Hydrosob equipment in room HYD-003 ("powder room"). A NIOSH-certified dust mask was used to perform the unjamming operations.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Employees working in the Hydrosorb room (room #4) were exposed to caught-in and crush-by hazards and the employer did not ensure that all the machines are properly guarded: a. The Hydrosorb operator tightening the belt into the pulley of the winding system before a new roll is started was exposed to a caught-in hazard. b. The two winding systems used to roll the finished filter product from "Alcohol Monitor" (west wall) creates an in-going nip point from moving rollers that have not been effectively guarded.
