29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access. a) 812 - 814 Rainbow Dr. Guilderland NY. - On or about December 29th, 2014 - Employees were exposed to fall hazards of 13 to 15 feet in height while using an extension ladder to access a roof bracket scaffold. The ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access.
