29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. a) Pump Jack Scaffolding at Exterior Building Front: Employees engaged in removal of siding and gutters, were exposed to a fall hazard of approximately 16 feet while working from the platform of a pump jack scaffold and the sides of the scaffold was not protected by a standard guardrail system, on or about 12/29/14. b) Pump Jack Scaffolding at Exterior Building Front: Employees engaged in removal of siding and gutters, were exposed to a fall hazard of approximately 16 feet while working from the platform of a pump jack scaffold and the scaffold was not protected by a standard midrail system, on or about 12/29/14.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Pump Jack Scaffolding at Exterior Building Front: Employees engaged in removal of siding and gutters, were using an aluminum extension ladder extended to 16 feet above the ground surface to access the scaffold's work platform and the side rails did not extend at least 3 feet above the upper landing of the scaffold, on or about 12/29/14.
