29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  a. On or about December 31, 2014, employees were exposed to the hazard of falling while working from a man-basket which did not have the safety chains in place and the employees were not using a personal fall protection system.

29 CFR 1926.602(c)(1)(viii)(A): The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel:   a. On or about December 31, 2014, employees were working from a man-basket which was not secured to the forks or lifting carriage of the powered industrial truck that was being used to raise and support the man-basket.
