29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. Employees from Trinity Packaging, L.L.C. were not protected by eye and face protection when exposed to liquid chemicals and vapors while changing out propane tanks on the companys forklift.

29 CFR 1910.138(a): General requirements. The Employer did not select and require employees to use appropriate hand protection when employees' hands are exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes. Employees from Trinity Packaging, L.L.C. were not protected by gloves where there was a possibility of thermal burns from skin contact with liquid propane during fuel tank changing procedures.

29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer did not ensure that each operator had successfully completed the training. Trinity Packaging L.L.C. had not ensured that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training. The owner, warehouse manager and one employee were operating a powered industrial truck, serial #AF82F03279, without successfully completing training.
