29 CFR 1910.132(a): Protective equipment was not maintained in a sanitary and reliable condition:       The company did not provide reliable protective equipment for fall protection due to the use of expired DB Sala Layard Model Number 1201106, and the non-approved use of a Guardian Harnesses with the DB Sala Lanyard.    Employees were exposed to fall over 25 feet from a lower level while conducting service work on legs of the facility.

29 CFR 1910.303(e)(1)(i):  Electric equipment shall not be used unless markings have been placed on equipment such as the manufacturer's name, trademark, or other descriptive marking by which the organization responsible for the product may be identified:       An electrical box in the milling room did not have markings to identify manufacturers name, trademark, or other descriptive markings.  The employees working in the milling room are exposed to possible combustible dust hazards from grain legs feeding processing equipment.
