On or about January 5, 2015, employees were exposed to struck-by and engulfment hazards when the employees were working in a trench approximately 10-14 feet deep installing a manhole.  The walls of the excavation were not sloped or benched to 45 degrees prior to backfilling.  There was no trench box on site.
