29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed: (a) 2811 S Pennsylvania Ave., Long Beach Twsp., NJ: On or about 12/31/2014, two employees of John Harquist were installing second story windows on a single family residence. The employees were using an elevated working platform that was supported by two aluminum dual-sided access A frame step ladders. The step ladders were not designed to be used in this manner. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1053(b)(6): Unsecured ladder(s) were used on surfaces that were not stable and level: (a) 2811 S Pennsylvania Ave., Long Beach Twsp., NJ: On or about 12/31/2014, two employees of John Harquist were installing second story windows on a single family residence. The employees were using an elevated working platform that was supported by two aluminum dual-sided access A frame step ladders. The surface below the step ladders was not level and the step ladders were not secured resulting in one of the ladders falling over. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
