29 CFR 1910.133(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.    On or about January 7, 2015, the employer did not require eye or face protection for employees to strip old wax from tile floors located on the 1st floor of the Old Hyper building by utilizing hand scraping tools and a chemical stripper - Devastator- at full strength(11-12pH Corrosive).

29 CFR 1910.1001(j)(1)(iii): Employers shall include asbestos in the hazard communication program established to comply with the HCS (� 1910.1200).     On or about January 7, 2015, WFF Facility Services' hazard communication program did not meet the requirements for compliance with the HCS (� 1910.1200). WFF Facility Services was knowledgeable that floor tiles within the Old Hyer building contained chrysotile asbestos.

29 CFR 1910.1001(j)(3)(iii): Building and facility owners shall inform employers of employees, and employers shall inform employees who will perform housekeeping activities in areas which contain ACM and/or PACM of the presence and location of ACM and/or PACM in such areas which may be contacted during such activities.    WFF Facility Services did not inform floor care/project technicians of the presence of chrysotile asbestos within floor tiles in the Old Hyper building.  On or about January 7, 2015, WFF Facility Services employees were required to strip and wax floor tiles in the Old Hyper building.

29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.    On or about January 7, 2015, WFF Facility Services did not assess for hazards the project in the Old Hyer building for stripping and waxing chrysotile asbestos containing tile floors while utilizing Devastator a corrosive. A signed certification of hazard assessment was not provided upon CSHO request.
